Over the last 15 years, we gained great insight into ACS in Malaysia \u2013 including patient profiles, clinical presentations, clinical management, and prognosis.\u003C\/p\u003E\r\n\r\n\u003Cp style=\"text-align:justify\"\u003EThis Report is unique, capturing data during the COVID-19 pandemic, illustrating the commitment of centres across Malaysia that continued to provide care for ACS during this challenging time. Also, it demonstrates the perseverance to continue contributing data to this Registry, from which this Report is built on.\u003C\/p\u003E\r\n\r\n\u003Cp style=\"text-align:justify\"\u003EAs we emerge from the COVID-19 pandemic, valuable lessons were learnt, notwithstanding the way we manage ACS cases during and after the pandemic. The NCVD-ACS has been very much a part of this learning journey \u2013 generating data, valuable clinical information, and peer-reviewed publications. Data from the NCVD ACS Registries are now incorporated into National Clinical Practice Guidelines, making them more relevant to the local clinical setting.\u003C\/p\u003E\r\n\r\n\u003Cp style=\"text-align:justify\"\u003EThis Report continues to reflect the immense work from across Malaysia, from all the contributing source data providers (SDPs) \u2013 sites from both public and private sectors. I would like to thank Prof Dr Wan Azman and all those who contributed time and energy, beyond their clinical duties, to put the Report together. Today, we are proud to see this registry grown and continue to bear fruits!\u003C\/p\u003E\r\n\r\n\u003Cp\u003EThe first three reports accumulated data of our PCI experience from 2007 to 2014; it included 38,595 patients, 41,997 procedures and 54,202 coronary lesions. From 2015\u20132016, we had 19,494 patients who underwent procedures and 14,815 (76%) patients with coronary lesions. In 2017\u20132018 report, it included 21,618 patients who underwent procedures, with 15,144 (70.1%) with coronary lesions.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EThis latest report (2019-2020) included 24,309 patients who underwent procedures, with 19,007 (78.2%) with coronary lesions.\u003Cbr \/\u003E\r\nWe hope the registry provides a \u201creal-world\u201d database of contemporary PCI practice in Malaysia. The registry is a guide to evaluate PCI outcomes based on selected performance measures; to determine the cost-effectiveness of PCI, and to determine the level of adherence to practice guidelines. The registry is also a platform to stimulate research; to facilitate quality improvement activities; to act as a reference for future studies; to facilitate research and development; and to benchmark with other national\/regional PCI registries such as ASPECT, ASEAN.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EWith commitment and teamwork, much has been achieved. However, much remains to be done. The way forward is to go beyond \u201coutput\u201d to \u201coutcome\u201d, and beyond \u201cquantity\u201d to \u201cquality\u201d. With the sizeable number of cases reported, we may now embark on more analysis on trends and subgroups, to determine the factors that contribute to procedural success and long-term patient-centred outcomes. To this end, we may embark in appraising our practice by referring to both clinical practice guidelines and appropriateness use criteria.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EBeyond its value in improving healthcare service, our registry has proven to be a platform for registry-based randomised controlled trials and post-marketing surveillance as required by the recent regulations for medical devices. This is integral in the ever-changing field of interventional cardiology.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EThe way forward for the coming decades will depend on the continuous commitment and support in leadership and funding from NHAM. The machinery of the NCVD registry lies with each of us. The NCVD-PCI registry will continue to be a voluntary collaborative group, with shared ownership. The NCVD-ACS has been very much a part of this journey \u2013 generating data, valuable clinical information and peer-reviewed publications, such that patients with ACS in Malaysia are experiencing improved clinical outcomes. Data from the NCVD ACS Registries is now incorporated into the National Clinical Practice Guidelines, making them more relevant to the local clinical setting.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EThis Report is the result of immense work from across Malaysia, from all the contributing source data providers (SDPs) \u2013 sites from both public and private sectors \u2013 under a voluntary reporting process. From that commitment, our team in the Heart House, and the Writing Committee led by Prof Dr Wan Azman Wan Ahmad, were able to generate this valuable Report. I would like to thank him and all those who contributed time and energy, beyond their clinical duties, to put these chapters together. We were able to see the trends in the way our patients present and the treatment they received. More importantly, with concerted efforts especially in getting our patients to undergo coronary angiogram and angioplasty, we have been able to reduce mortality, which is the hardest end-point. The number of primary PCIs have increased and even with thrombolytic therapy we see an improvement in a shorter door-to-needle time. I would like to postulate that these improvements are due to better knowledge and improvement in managing\u003Cbr \/\u003E\r\nprocesses in the Emergency Departments. It could only be done with knowing where we stand, achieved by having our NCVD-ACS.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EThis report was made possible primarily due to the many source data providers (SDPs) who have voluntarily contributed valuable information into our secured electronic database. The information obtained from SDPs across the country have not only provided a snapshot of ACS in Malaysia, but more significantly, uninterrupted annual data on the disease since 2006. We would like to acknowledge the contribution of our peers and colleagues in both the Governance Board of the NCVD and the Steering Committee of the NCVD-ACS Registry. We also commend the efforts of the NCVD Medical Writing Committee for this report, led by Prof Dr. Wan Azman Wan Ahmad, who has put in over a decade of work into all the reports of the NCVD-ACS Registry published to date. Such data is crucial as it represents facts rather than the usual estimates generated for the practicing healthcare professionals, private and public hospital administrators, policy makers, patients advocates, as well as the pharmaceutical and insurance industries. In addition to being an academic reference, the report can be used to improve quality of care, delivery of services, and healthcare planning.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EA vast amount of data was collected, and this was only made possible with the countless hours the medical and nursing staff from individual sites throughout Malaysia spent uploading the information online. I thank them for their effort, commitment, perseverance, and enthusiasm for the last ten years; they have truly gone beyond the call of duty.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EOur sincere gratitude to the writing committee headed by Prof Dr Wan Azman Wan Ahmad, which devoted many weekends pouring through the data, analysing the figures, and transforming the data into meaningful information. The volume of data compiled reflects the dedication of all those involved at the SDPs, staff of the National Heart Association of Malaysia (NHAM) and those involved from the Ministry of Health (MOH), who have been consistent in their efforts to ensure the data collected remained of the highest quality. There has been a year-on-year increase in thenumber of patient notification reports into the NCVD-ACS Registry; In 2015, for the first time, we crossed the 10,000 patients mark!\u003C\/p\u003E\r\n\r\n\u003Cp\u003EOn behalf of the Steering Committee of the NCVD-ACS Registry, I would like to thank all those who contributed to the writing of this report and the Governance Board of the NCVD for their support. This report also examines the similarities and differences among the patients with ACS admitted between 2014\u20132015 and those described in the previous report.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EOver the last decade, deaths from circulatory diseases including ACS, remains the principal cause of mortality in the country. Data from previous NCVD-ACS Registry reports have been published and have directly contributed to our National Clinical Practice Guidelines (CPGs). Through the NCVD-ACS Registry reports, we have seen the impact of CPGs and the subsequent reduction in in-hospital and 30-day all-cause mortality rates.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EFinally, I would like to acknowledge Prof Datuk Paduka Dr Wan Azman and Prof Datuk Dr Sim Kui Hian for their vision and leadership over the last decade and for their direct involvement in this NCVD Registry. It is a culmination of sustained dedication and commitment of many people whom we are proud to acknowledge on behalf of the National Heart Association of Malaysia (NHAM).\u003C\/p\u003E\r\n\r\n\u003Cp\u003EOver the years, the number of hospitals involved in the registry has steadily increased and we now have 22 hospitals involved. We are also in the process of involving private hospitals in the registry.\u003C\/p\u003E\r\n\r\n\u003Cp\u003ESince we started in 2006, we have managed to collect important data from 31,635 patients in the management of Acute Coronary Syndrome (ACS). From our registry, we know that the disease trend is alarming, involving younger patients with multiple co-morbidities. This has enabled us to get a clearer picture of the profile of patients presenting with ACS in our country. With this information we can better plan for improvements in treating this number one killer in our country.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EThe Ministry of Health Malaysia and the NHAM have been providing support and funding for this to continue. This report gives us an opportunity to compare the baseline characteristics, presentation, lesion complexity, device use and clinical outcome between these two periods 2007-2009 and 2010-2012. The mean age for PCI remains approximately the same i.e. 57 years. Our PCI population is still younger than in the Western countries. Of concern 72% have three or more cardiovascular risk factors. Males still predominate females by 4:1.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EIn this report, more procedures were for stable coronary artery disease (CAD) as compared to acute coronary syndrome (65.2% vs. 56.4%). This may not be a true reflection as some stable CAD cases were PCI for patients post AMI. The door to balloon time has remained the same with a median of 90 min but what appears troubling is the delay in transferring patients to a hospital with catheterization lab facilities (180 min in 2007-2009 vs. 220 min in 2010-2012).\u003C\/p\u003E\r\n\r\n\u003Cp\u003EWe continue to perform a high number of complex lesions (Type B2 and C lesions: 58.8% in 2007- 2009 vs. 61.6% in 2010-2012).The use of drug eluting stent (DES) has increased over the years from 53.0% to 64.0%, reflecting the change in practice and possibly due to reduced cost of DES.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EThe procedural success rate remains high (96.8% vs. 95.3%). Over this period, the number of hospitals involved in the registry has increased and we now have 16 hospitals. However there is still no involvement yet from the private hospitals. The continuing challenge appears to be to sustain the number of data entries from each participating hospital. In some centers, the numbers have increased, whilst in others it has actually decreased. Part of the reason is due to lack of manpower in participating centers to key-in data. The number of follow-up is also decreasing.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EIt cannot be overstated that such a database is very important for any nation particularly that pertaining to coronary artery disease. As it is, the picture beginning to emerge appears rather alarming. Apart from the fact that is still the number one killer in our country, it appears to affect an even younger proportion of our population. More worrying is that the number of women having this disease is also on the rise. However, it may be just a presumption. Therefore, it is hoped that this registry will give us a better idea of the groups most at risk and also the pattern of disease. This will enable us to plan the necessary steps to mitigate its impact on our society. The National Heart Association of Malaysia (NHAM) is fully aware of this registry\u2019s importance and continues to fund the ACS registry.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EThe WHO and United Nations has already recognized this problem and has undertaken several initiatives for various countries to adopt in combating this disease. However, the cornerstone of any strategy is still data. For coronary artery disease, this (ACS registry) together with the PCI registry forms the backbone of our information. Last year, we have improved on the data entry forms and have appointed study coordinators in various centres. We hope that this will improve the quality and amount of information submitted to the database. However commitment and participation are the two main ingredients of a successful registry.\u003C\/p\u003E\r\n\r\n\u003Cp\u003EOn behalf of the NCVD-ACS committee, we are grateful to many people in making this registry a reality. Not least the Ministry of Health, the participating centers, the doctors, nurses and paramedics involved in data collection, entry and the writing committees for their tireless efforts. It is hoped that in future, more centres, and the number of individuals involved in this project including the writing committee members, will come forward. Despite these advances, however, it is clear that evidence is unevenly translated into clinical practice. In virtually all countries and practice settings where it has been measured, there are significant gaps between guideline recommendations and actual clinical care. In the United States, the Institute of Medicine has said there is a \"quality chasm\" in healthcare, and has strongly advocated the need to make healthcare delivery more effective, safe, equitable, timely, efficient, and patient-centered.\u003Cbr \/\u003E\r\n\u003Cbr \/\u003E\r\nTo achieve these aims of higher quality of care, a critical first step is to measure how care is currently being delivered. Only in this way, can gaps in care delivery be identified and targeted quality improvement interventions be implemented. In this way, quality improvement itself becomes \"evidence-based\", and is more likely to lead to substantive improvements in patient outcomes.\u003Cbr \/\u003E\r\n\u003Cbr \/\u003E\r\nWith this context, the Malaysian Acute Coronary Syndrome Registry continues to be a tremendous accomplishment. This is a world-leading effort demonstrating the importance of clinical registries as a facilitator of improving quality of care and patient outcomes. The data reported here should lead to targeted local and national quality improvement efforts for ACS care in Malaysia. The ongoing existence of the registry will allow tracking of the impact of these quality improvement efforts on processes of care and patient outcomes.
